If your Privacy Policy includes a reference identification number in the upper right section of the privacy notice. The four-digit validation code is also referenced on the notice.

By Policy Number

The policy number is located on your customer communications. Enter the number exactly as shown.

If you hold a policy or account jointly with someone else, your opt-out choices will apply to everyone on the account.
